Use this form to send product gifting requests to a selected celebrity.

• Product Type: Enter the product category (e.g., Tops, Shoes, Accessories, Skincare)
• Number of Items: Enter a quantity between 1–10

🔹 Brand Information

• PIC (Person in Charge): Name of your contact person
• PIC Contact: Email or phone number
• Social Media Account: Add one or more brand SNS accounts
• Website: Official brand website URL

• Product Delivery Method:
  • Deliver to agency
